Payment Agreement & Cancellation Policy
Our patients are very important to us. We take pleasure in reserving a special amount of time for each patient so that we can provide the highest quality care. In order to serve each of our patients in the best manner, we need to carefully arrange our schedule. Please read the following agreement. It explains our financial agreements and our policies regarding cancellations while you are under our care.
Payment is always due at the time of service.
We accept the following forms of payment:
Cash, Credit Card, Debit Card, Care Credit, and LendingClub.
We do not accept insurance, however:
We will be happy to prepare a dental insurance claim that you can submit to your insurance company to request reimbursement of your charges.
Cancellation Policy
No Shows and Late Cancellations of less than 2 business days’ notice:
New Patients.
$120 missed appointment fee will apply.
Hygiene Patients.
$120 missed appointment fee will apply..
Note: Patients scheduled for dental work with Dr. Zraigat will have a minimum $200.00 missed appointment fee or 20% of the sche
